Andrew C. Castrodale, M.D.
Family Practice/Obstetrics (OB)
Dr. Castrodale arrived at Coulee Medical Center in 1998. He focuses on the complete scope of family medicine with fellowship training in obstetrics. He attended the University of Washington for his bachelor's degree and his medical degree. He completed his Family Medicine Residency and Obstetrics Fellowship at the Sacred Heart and Deaconess Hospitals in Spokane.

Jacob Chaffee, M.D.

Dr. Chaffee joined Coulee Medical Center with emphasis on obstetrics, emergency medicine, trauma, cardiovascular disease and diabetes. He was born in Seattle and raised in Arlington. Dr. Chaffee graduated from the University of Washington, served his internship in Spokane and his residency work in Colville, WA.

Eric Dudenhofer, ARNP, FNP-C
Emergency Medicine
Eric joined Coulee Medical Center in 2012 and has experience in emergency medicine and dermatology. He received his Bachelor of Science in Nursing from Whitworth College, graduated from Washington State University in 1999 with a Masters in Nursing, and is a certified family nurse practitioner.

Anna Hardesty
Physical Therapist
Anna Hardesty joined the Coulee Medical Center team in February of 2020. She attended Andrews University in Michigan for her undergrad and graduate degrees, graduating with her Doctorate of Physical Therapy in August of 2015. Her first job out of school was in Kent, WA at ATI Physical Therapy – working for almost three years as an outpatient physical therapist. She then became a travel physical therapist for almost 2 years before signing on with Coulee Medical Center. She currently works in both the outpatient clinic and inpatient/LTC settings here at Coulee Medical Center.

Cody Ellefsen, D.O.
Emergency Medicine
Dr. Ellefsen came to Coulee Medical Center from Spokane, WA where he completed his Residency at Family Medicine Spokane in 2009. He received his B.S. in Sports Medicine and Athletic Training in 2002. He attended the Arizona College of Osteopathic Medicine at Midwestern University in Glendale, Arizona where he received his Doctorate of Osteopathic medicine in 2006. Dr. Ellefsen has been with Coulee Medical Center for many years and continues to work in our Emergency Department.

Elizabeth Hsu, MD
Endocrinology/Obesity Medicine
Dr. Hsu joined the Coulee Medical Center Team in November of 2015. Born in New Zealand, she graduated from the Royal College of Surgeons in Ireland, Dublin, Ireland and her main focus areas are Endocrinology, general diabetes and thyroid issues. Her Residency was completed at the University of Texas Medical Branch, Galveston, TX and her Fellowship for endocrinology was at the Houston Methodist Hospital, Houston, TX.

Wendy Hughes, ARNP
Family Practice/Behavioral Health
Raised in Wenatchee and residing in the Coulee area, Wendy received her Master's from Washington State University and she is a certified family nurse practitioner. In 2021, Wendy received her Post-Graduate Certificate in Psychiatric Mental Health from Frontier Nursing University. Wendy is board certified in family practice by the American Academy of Nurse Practitioners and is board certified in behavioral health by the American Nurses Credentialing Center. Wendy splits her time between Coulee Family Medicine and Coulee City Clinic.

Jennifer Knox, M.D.
Family Practice/Obstetrics (OB)
Dr. Knox has experience and education in family medicine, obstetrics, inpatient, and outpatient care and worked in the research field for four years before deciding to pursue her medical degree at the University of Washington. She’s a mother of two and excited to be joining CMC’s family medicine and obstetrics team.

Rick King, P.T.
Physical Therapist
Rick was raised in Index, WA in the Cascade Mountains on the west side of Stevens Pass. He graduated from Eastern Washington University's Physical Therapy program in 1988. After practicing in Spokane for 3 years, he moved to Grand Coulee in 1991 and opened Grand Coulee Physical Therapy. After 23 years, Rick became a full-time CMC employee in 2014.

Brian Lee, CRNA
Anesthesiology
Brian joined Coulee Medical Center in 2019 from Spokane, WA where he has been providing anesthesia services since 2015. He received his Bachelor of Science degree from Kettering College of Medical Arts in Ohio and graduated with a Master of Anesthesia Education from Gonzaga University in 2015. He completed his anesthesia residency at Sacred Heart and Holy Family Medical Centers in Spokane, WA. His anesthesia practice includes general surgery, emergency/trauma, ENT, urology, neurosurgery, vascular, orthopedic, high-risk obstetrics, and pediatrics.

Alvina Marris, PhD
Psychologist/Behavioral Health
Dr. Alvina Marris is a Colville Tribal member who grew up in the Grand Coulee Dam area and graduated from Lake Roosevelt High School. She obtained her bachelor's degree in psychology from Eastern Washington University. Dr. Marris attended Oklahoma State University and received her Master's and Ph.D. in Clinical Psychology in 2012. She has experience working with children and adults, addressing most major mood disorders. Dr. Marris is excited to join the Collaborative Care team working as the Behavioral Health Care Manager.

Chris C. McGlothlen, PA-C
Emergency Medicine
Chris McGlothlen, PA-C was born and raised in Yakima, Washington. He joined the Coulee Medical Center team in October of 2017. Chris graduated from the University of Washington in Seattle, Washington with a B.S. in Atmospheric Sciences in 1990. He then served as an Officer in the U.S. Navy. Chris went on to obtain his B. S. in Physician Assistant Studies from Oregon Health & Sciences University in Portland, Oregon in 1997. Chris specializes in Emergency Medicine.

Charles Randolph, M.D.
Family Medicine/Obstetrics (OB)
Dr. Randolph studied Biology at Biola University then went to medical school at Loma Linda University, obtaining his MD in 2019. He completed his family medicine residency in Renton, WA. He and his wife and two children are now happy to call Grand Coulee home. His areas of interest include broad-spectrum primary care, obstetrics, global health, cross-cultural Medicine, and improving health education.

Karen came to Coulee Medical Center from Northeast Washington Medical Group in Colville. She received her bachelor's of science in nursing from Minot State University in Minot, North Dakota, in 2000 and her Master of Science, Family Nurse Practitioner from the University of Minnesota in 2002.

Michael Rimpler, CRNA
Anesthesiology
Mike joined Coulee Medical Center in 1999. He received his M.S. in Biology from Southern Connecticut State University and his board certification in anesthesia from Bridgeport Hospital Nurse Anesthesia program. His anesthesia practice has included open heart surgery, trauma, high-risk obstetrics, pediatrics, and regional, vascular, thoracic, neuro and ambulatory surgery.

Cassandra Viramontes, MSN, ARNP, FNP-C, CEN, SANE
Family Practice
Cassandra previously worked as a Registered Nurse for 14 years, 12 of those as a critical care nurse working in Emergency and Intensive Care. Educating, managing, and working closely with staff and patients during that time, she realized the increasing need for compassionate, comprehensive primary care. Cassandra joined Coulee Medical Center in 2021 with the goal of making outstanding medical care more available to our community.
