$20The TSH Test can help detect whether the thyroid gland is working properly and whether it is overactive (hyperthyroidism) or underactive (hypothyroidism). It can also detect a thyroid disorder before symptoms appear.Iron Panel (Transferrin and Iron)
$20The Iron Panel measures the levels of iron in your blood. This test also includes a transferrin test, which measures transferring, a protein that moves iron throughout the body.PSA Screen (Prostate Cancer Detection – over 40 years of age)
$20The PSA Screen is a blood test that measures the amount of PSA in the blood. PSA is a protein produced by the prostate, which is part of the male reproductive and urinary systems. This test is primarily used to screen for prostate caner, but it can also detect other condition that increase PSA levels. This test is recommended for individuals 40 years of age and older.Vitamin D
$20A vitamin D test measures the levels of vitamin D in your blood. Vitamin D helps your body absorb calcium to build healthy bones and teeth. It also helps keep your muscles, nerves, and immune system working normally.
Please do not eat or drink anything except water 12 hours prior to blood draw. Medications should be taken as usual.

- These tests DO NOT replace an annual exam, please follow up with your provider.
- Test results will NOT be sent to your provider.
- Test results will ONLY be mailed to the person at the mailing address listed on the form.
- If you wish for your provider to have the results, you will need to personally make an appointment with the provider to review your test results.
